I think that here in Spain that happens very little and not because the error does not exist but because it must be the most underused feature of the iPhone.
To this day I have only tried it 1 time and it is that despite the many sold, it is not the most widespread among the Spanish population so it is not easy to coincide with any of your friends who also have it. Also, since it is not compatible with other platforms, it leaves you few options.

Black screen with facetime sometime but I suppose it will be something normal coming from a wifi network, soon I stopped using it, there are more people with Skype than with iPhone 4 and because of that Skype it is not the best but it is quite acceptable, but photos because if not the truth.
The black screen has never happened to me, but something else curious happens to me. I have an iPhone 4 and my girlfriend has an iPod touch 4G. Well, at first we started calling each other and sometimes the other idevice didn't start ringing. We were angry for a while because we could not always call each other. The funny thing is that when she opened the phone application or she opened the face time application, the 3 or 4 calls suddenly came to us that we had tried to make as if they were missed calls all in a row. Soon we discovered that so that calls always come to us by face time I have to have the phone app in multitasking and she the face time or otherwise the calls do not arrive.
I have to add that the video and sound quality is very good and it is normal that it is not used much between friends because video calls are more familiar, I have friends with iPone 4 and I have never called them for face time.

What has happened to me is that by calling my mother, she has seen on her screen the photo that I have of her in my phone contact. It happened two days ago.
It has happened to me several times to receive a call marked as voicemail and the screen does not turn on. And when I have looked at what it was I have not found anything, no missed call, no sms message, no email, ... no idea.
